Anticoagulation

The Anticoagulation Monitoring Service (AMS) Inpatient Package is a required rotational learning experience. It is designed to enhance the resident's therapeutic knowledge base and promote critical thinking skills while providing a safe and efficacious process for the care of patients receiving anticoagulants. The resident will demonstrate understanding of pharmacology/pharmacokinetic parameters of antithrombotic agents and current practice parameters by initial patient assessment, determining appropriateness of therapy based on established criteria for the service, developing patient specific care plans with goals, providing patient education, monitoring therapy progress, and providing follow up while the patient is on service.

The Anticoagulation Monitoring Service (AMS) Outpatient Package is a required rotational learning experience. It is designed to enhance the resident's therapeutic knowledge base and promote critical thinking skills while providing a safe and efficacious process for the care of patients receiving anticoagulants. The resident will become familiar with the policies and procedures for anticoagulant management in the AMS Clinic. The resident will review medication histories, conduct patient monitoring (blood pressure, heart rate, and INRs), assess compliance to therapy, evaluate drug-food/drug-drug interactions, and provide recommendations to the preceptors regarding changes to therapy and return visits. At the end of this rotation, the resident should be prepared to see patients independently in the clinic and make recommendations for therapy during the weekly longitudinal rotation, with minimal advice from the preceptors on checkout.

Antimicrobial Stewardship

The Antimicrobial Stewardship Package is a 6-week elective learning experience offered to PGY1 residents at Saint Thomas West Hospital.  It is designed to enhance the resident's therapeutic knowledge base and promote both critical and independent thinking skills while providing care to hospitalized patients. The Antimicrobial Stewardship Team is composed of an Infectious Disease Physician and Clinical Pharmacist.  The resident will help manage patients followed by the Antimicrobial Stewardship Program.  The resident is responsible for identifying and resolving medication therapy issues for patients being followed by the Antimicrobial Stewardship Program.  In addition to conducting surveillance activities, the resident will gain experience analyzing antimicrobial utilization throughout the system and developing strategies to promote the rational, safe, and cost-effective use of antimicrobials, through both data-mining and meeting attendance.  The resident must provide education to healthcare providers and personnel as the need arises.

Cardiology

The Cardiology Package is a 6-week elective rotational learning experience. It is designed to enhance the resident's therapeutic knowledge base in cardiology, specifically heart failure, and problem solving skills while providing a safe and cost-effective process for the care of patients both inpatient and outpatient.

Critical Care

We currently offer two different Critical Care Packages: Medical Intensive Care Unit (MICU) and an integrated ICU experience consisting of Cardiovascular, Neurologic , and Surgical Intensive Care Units (CVICU, NICU, & SICU). The Critical Care Packages are a 6-week elective rotational learning experience. They are designed to enhance the resident's therapeutic knowledge base and promote critical thinking skills while providing care to critical care patients in a variety of settings. The resident will establish professional relationships with members of the multi-disciplinary team through daily critical care rounds in one of two 44-bed intensive care units. Both learning experiences will include, but are not limited to discussion of critical care topics, providing assistance with drug information questions, development of the resident's precepting and mentoring skills, project participation and attendance of Critical Care Committee meetings.

Management

The Practice Management/Medication Safety Package is a 6-week required rotational learning experience. It is designed to focus on a review of the director of pharmacy role and responsibilities with a current literature review, role playing, shadowing with direct involvement that will include: Discussions on Budgeting/Finances, Human Resources, Regulatory Agencies, Risk Management, Development/Updating a Policy and Procedure, Problem/Conflict Resolution, Attending and Participating in Hospital Committees, Study and Promotion of Medication Safety in relation to current and future standards of the Joint Commission/National Patient Safety Goals, Institute of Medicine, Leapfrog and Institute for Healthcare Improvement.

Nuclear Medicine

The Nuclear Medicine Package is an elective rotational learning experience. It is designed to teach residents the process of preparation and dispensation of radiopharmaceuticals according to existing standards of practice and the organization's policies/procedures. The experience will involve understanding handling precautions for radioactive material, acknowledging the different radioactive agents available, and assisting in drug information questions.

Nutrition

The Nutrition Support Package is a 6-week required rotational learning experience. This experience is designed to foster the development of or build upon skills necessary to the provision of parenteral and enteral nutrition. Upon consultation, the pharmacy practice resident will assess the need for, formulate, prescribe, and monitor specialized nutrition support based on current practice guidelines. The resident will gain experience in working closely with other practitioners, serving as an important member of the health care team.

Oncology

The Oncology/Investigational Research Package is a 6-week elective rotational learning experience. It is designed to enhance the resident’s therapeutic knowledge base and promote critical thinking skills while providing care to oncology patients. The resident will demonstrate an understanding of the use of investigational drug products according to regulatory requirements and established protocols. The resident will establish professional relationships with members of the health care team through daily oncology rounds, assistance in drug information questions, and participation in Tumor Conference, Hem/Onc/Rad Division, and Cancer Committee.

Operating Room Satellite Pharmacy

The OR Pharmacy Package is an elective concentrated learning experience. It is designed to enhance knowledge and develop skills required to operate a satellite pharmacy in the OR. The resident will compare and contrast medications used in the general OR setting versus the cardiac OR. The experience may involve compounding drips, refilling tackle boxes, performing audits for each case, and assisting with drug information questions.

Operations

The Operations Package is a 1-week required concentrated learning experience. It is designed to teach residents the process of preparation and dispensation of medications according to existing standards of practice and the organizations policies and procedures. The resident will perform necessary aspects of the medication-use system by preventing, identifying, and resolving pharmacy related patient care problems.

Orientation

The Orientation Package is a 5-week required rotational learning experience. It is designed to orient the resident to departmental policies, provide an overview of the residency schedule, explain the learning experiences and evaluation process, and assess personal goals. The resident will undergo computer based training involving Health and Safety, Corporate Compliance, and HIPAA. The learning experience is also designed to train residents the functions of the renal dosing service, IV to PO service, pharmacokinetic monitoring service, and consult service. At the end of orientation, the resident should demonstrate ownership and responsibility of the services for patients assigned.

Palliative Care

The Palliative Care Package is a 6-week elective rotational learning experience. It is designed to enhance the knowledge and skills particular to practice in the hospice care environment. The resident will become familiar with the hospice consult process and identify patients who are appropriate candidates through discussions with the physician and nurse practitioner. The resident will observe inpatient units and home care visits to gain an understanding of the medication and non-medication issues that relate to treating a Hospice patient.

Psychiatric Medicine

The Psychiatric Medicine Package is a 6-week elective rotational learning experience. It is designed to enhance the resident's clinical skills as they relate to the psychiatric patient. During the learning experience the resident will round with the physician on patients admitted to the Geriatric Psychiatric Unit.  The resident will also attend rounds with the nurse practitioner to evaluate inpatient psych consults.  Rounding may include attending patient and family care conferences with the health care team.  Additional activities include delirium consults, depression screening, home medication consults, and in-services on new antipsychotic medications.  The resident will be responsible for all medication-related patient care issues. 

Transplant

The Transplant Package is a 6-week elective rotational learning experience. It is designed to enhance the resident's therapeutic knowledge base and promote critical thinking skills while providing care to solid organ transplant patients, including Heart, Kidney, and Liver transplant patients. The resident will establish professional relationships with members of the transplant team through daily rounds. This learning experience will include, but is not limited to discussion of transplant topics, providing assistance with drug information questions, development of the resident's precepting and mentoring skills, project participation and attendance of multi-disciplinary meetings.
